As the patient, you are responsible to know and understand your insurance policy. The contract for health insurance coverage is between you the patient and your insurance company. As well, it is the patient’s responsibility to pay any co-insurance, deductibles, co-payments and any non-covered items as required by your health insurance carrier contract. All co-payments are to be paid at the time of service. We accept cash and checks. No credit cards or bank cards accepted.
